Main Areas of Responsibility:
The case manager will provide comprehensive assessment to determine eligibility of programs; develop client service plans; monitor and document client progress; prepare reports, files, and advocates for clients as needed; coordinate case management services with community agencies and other internal programs; monitor and document clients' progress; and prepare case notes and official reports.
Additional Areas of Responsibility:
This position prefers a BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; one year minimum of experience working with under-served populations required, intermediate computer skills in MS Office and Windows; and knowledge of Albuquerque social service systems. Bilingual in English/Spanish and multicultural experience is preferred.
