Case Manager

Type of Job: 
Main Areas of Responsibility: 

You will provide comprehensive assessment to determine eligibility of programs; develop client service plans; monitor and document client progress; prepares reports, files, and advocates for clients as needed; and coordinate case management services with community agencies and other internal programs; monitors.

Additional Areas of Responsibility: 

You will also document clients' progress and prepare case notes and official reports; as well as outreach to English and Spanish speakers.

Other Information/Requirements: 

Prefers a BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; one year minimum of experience working with under-served populations, intermediate computer skills in MS Office and Windows; and knowledge of Alb. social service systems. Bilingual in English/Spanish is required; and multicultural experience is preferred.
