COVID-19 Care Coordinator

Main Areas of Responsibility: 

Under the direction of the Clinical Programs and Infection Control Manager, the COVID-19 Care Coordinator works closely with LFMC patients and staff and participates as a member of the LFMC vaccination program team to implement LFMC-hosted vaccination clinics for LFMC patients, staff, and employees of approved partner organizations.

Additional Areas of Responsibility: 

Collaborate with the Clinical Programs Manager and EMR System Administrator to identify eligible populations and target outreach numbers, and to coordinate outreach to patients; receive calls for COVID-19 Vaccination campaigns and collaborate with leaders from nursing & operations to schedule patients for 1st and 2nd doses of COVID-19 Vaccines; and schedule and document patient contact and vaccinations in the patient’s medical record.

Other Information/Requirements: 

High School diploma/equivalent. Candidate with experience in one of the following areas preferred: medical or dental front office work, inventory management, customer service, event planning, medical assistants, community health workers, patient navigation or care coordination, etc.; and fluent in both English and Spanish, preferred.
