Main Areas of Responsibility:
Under the direction of the Medicare Program Coordinator, the Medicare Program Nurse effectively interacts and engages patients at La Familia Medical Center (LFMC) providing case management, advocacy, and Medicare information and documentation for LFMC patients, including maintaining Medicare patient medical records and data for reporting.
Additional Areas of Responsibility:
Provides in-depth case management services to Medicare patients, with special attention and understanding of the needs and circumstances related to barriers to health care and the psychosocial support needed to cope with chronic, acute or terminal illness; carries out Case Management within the context of on-going needs, care and/or treatment planning, and monitoring; and advocates for clients and patients to resolve crises.
Registered Nurse licensure, High School diploma/equivalent and minimum of two years of experience required; basic knowledge and ability of general office procedures including filing, copying, faxing, use of a computer, to meet established deadlines, work under the direction of a supervisor and follow instructions for work completion, and take initiative to resolve patient concerns and problems; and fluent in both English and Spanish, preferred.
