Main Areas of Responsibility:
Provide comprehensive assessment to determine eligibility of programs; develops client service plans; monitors and documents client progress, prepares reports, files, and advocates for clients as needed; and coordinates case management services with community agencies and other internal programs.
Additional Areas of Responsibility:
Monitors and documents clients' progress and prepares case notes and official reports; outreach to English and Spanish speakers. This position prefers a BA/BS in Social, Behavioral, or Health Sciences and/or relevant experience in related field; 1 year minimum of experience working with under-served populations.
Intermediate computer skills in MS Office and Windows; and knowledge of Albuquerque social service systems. Bilingual in English/Spanish and multicultural experience is preferred.
