Case Manager - Housing

Job Description
Type of Job: 
Main Areas of Responsibility: 

The Case Manager - Housing establishes relationships with each family in order to support and stabilize the family, and coordinates community resources to help them become re-housed and self-sufficient.

Additional Areas of Responsibility: 

• Oversees housing-first based programs and homelessness prevention programs
• Conducts intakes
• Assists applicants in obtaining and maintaining stable housing
• Maintains ongoing communication including developing a Family Success Plan for each family
• Meets with participants to review service plan goals and conduct assessments
• Ensures compliance with contract requirements
• Compiles data and prepares reports

Other Information/Requirements: 

• Experience with family case management and home visiting programs
• Experience with housing programs
• High School Diploma or GED
• Bilingual English/Spanish
• Strong administrative skills, including organization and attention to detail
• Previous experience working with homeless, low-income, and at-risk populations
• Understanding of collaboration, teamwork, and advocacy skills
• Interpersonal skills
• Efficient in data entry & reporting
• Excellent verbal & written communication skills
