Main Areas of Responsibility:
Provides case management/referral services to Saranam families who have exited the traditional program; assists with strategy for exit transitioning; makes home/community visits; develops/facilitates after-care programming for families including leading focus groups, family input on program evaluation and community training opportunities for exited families; Interfaces with staff, volunteers, and outside agencies/organizations as appropriate for providing service and coordination of care.
Additional Areas of Responsibility:
Bilingual (English/Spanish) preferred.
Master’s in human service field preferred; bachelor’s required. Preference for current NM licensure (LMSW, LISW, LPCC, LMHC). Minimum 4 years' case management experience. Strong clinical background/experience working with homeless highly desirable. Familiarity with Albuquerque social services and resources preferred. Local travel home/site visitation required. NM driver’s license & personal vehicle required. Willing/able to work irregular hours and have a flexible work schedule
