Housing Case Manager

Type of Job: 
Main Areas of Responsibility: 

Work directly with formerly homeless individuals providing intensive case management services in a variety of settings including client homes, agency office space and occasional street outreach. The case manager will also work closely with resident services staff, housing management, landlords, and community partners to identify needs and coordinate resources including substance abuse referrals, mental health referrals, employment counseling, financial counseling and housing retention.

Additional Areas of Responsibility: 

Duties and Responsibilities: tenant support and advocacy, supportive services and community partnerships, program tracking and outcome measurements.

Other Information/Requirements: 

Minimum Requirements:
• High school diploma or GED with a minimum of 3-4 years’ experience working with the target population
• Bachelor’s degree in education, social work or related field plus 1 year experience working with the target population
• Computer literate
• Possession of valid driver’s license and good driving record in order to transport residents in agency vehicles, as needed
• Able to pass CYFD background check
• TB test
