Main Areas of Responsibility:
This position operates housing-first based programs and homelessness prevention programs; conducts intakes for housing program applicants; assists applicants in obtaining and maintaining stable housing; maintains ongoing communication including developing a Family Success Plan for each family; and meets with participants on a regular basis to review service plan goals and conduct assessments.
Additional Areas of Responsibility:
This position ensures that programs are compliant with contract requirements; and compiles data on a regular basis and prepares monthly, quarterly, and annual reports.
This position requires you to have experience with family case management and home visiting programs in a social services setting; a High School Diploma or GED equivalent; and to be bilingual English/Spanish.
