Housing Case Manager, Permanent Supportive Housing Program

Full-Time
Main Areas of Responsibility: 

Participant screenings/intakes
Housing placement: housing search, landlord screening, application process, inspections and move-in activities
Negotiate leases
Create a Housing Sustainability Plan for clients: budgeting, life skills, assist with entitlements
Conduct assessments/monitor client progress through visits/telepone contact
Provide crisis intervention if needed
Provide referrals to healthcare, behavioral health, employment, daycare resources
Maintain case notes/records

Additional Areas of Responsibility: 

Advise Director of participant progress or needs for support
Work with Director on program projections, management and development
Provide written notice of program completion to landlord/participant
Conduct discharges
Learn emergency procedures
Provide on-call shelter backup
Other duties as assigned

Other Information/Requirements: 

Proficienct in Microsoft Word and Excel
Effective written/oral communication skills
Ability to relate to people experiencing homelessness
First aid and CPR certified
Valid driver’s license and reliable transportation
Knowledge of community resources
Housing/case management experience desired
BA in counseling, social work, social sciences, or related field, or minimum of 2 years of case management or commensurate experience
Flexibility to conduct meetings evenings or weekends, as needed
