Main Areas of Responsibility:
Assesses the needs of at-risk youth (and their families when appropriate), as part of a clinical team, in order to advocate, arrange and coordinate a variety of wraparound services, and family team meetings that link them to quality healthcare, psychosocial, behavioral health services and other indicated community supports.
Additional Areas of Responsibility:
Improves overall client health, psychosocial and academic outcomes by providing culturally responsive, timely and coordinated care to address a wide range of presenting problems.
Bachelors’ degree and 3 years of relevant work experience or experience. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.
