Main Areas of Responsibility:
Under indirect supervision, provides a full range of medical and supportive case management services to patients and their families in order to improve quality of life and maintain the highest possible ability to function within the community. Functions as a member of and facilitator for the patient’s care team to ensure comprehensive coordination of medical and psychosocial services.
Masters’ degree or 30 credit hours towards a master's degree, preferably in a medical or social service field, with 3 years’ experience providing case management services, or a bachelor's degree, preferably in a medical or social service field, with a minimum of 5 years’ experience providing case management services. Verification of education and licensure will be required if selected for hire.
