Main Areas of Responsibility:
Under the direction of the Director of Patient Support Program, the Medical Case Manager works independently to provide long term coordination of care and services to patients in order to assist them to overcome the problems and needs associated with medical conditions, including addressing barriers to healthcare access.
Additional Areas of Responsibility:
The Medical Case Manager will conduct him or herself with integrity and professionalism consistent with workplace laws, and LFMC policy; coordinate with other departments, works effectively with peers, and contributes to organization objectives; maintain professional and technical knowledge by participating and completing LFMC training and external educational opportunities; and stays informed of policy, compliance and legal expectations affecting the department.
This position requires a Bachelor’s Degree in Social Work, and two years of experience in a related field, MSW preferred. In lieu of a degree, the incumbent must possess sufficient prior work experience in progressively more responsible positions during which the skills and abilities to perform all the essential functions have been amply demonstrated at a consistently competent level.
