Main Areas of Responsibility: 

Under the direction of the Medicare Program Coordinator, the Medicare Program Nurse effectively interacts and engages patients at La Familia Medical Center (LFMC) providing case management, advocacy, and Medicare information and documentation for LFMC patients, including maintaining Medicare patient medical records and data for reporting.

Additional Areas of Responsibility: 

Provides in-depth case management services to Medicare patients, with special attention and understanding of the needs and circumstances related to barriers to health care and the psychosocial support needed to cope with chronic, acute or terminal illness; carries out Case Management within the context of on-going needs, care and/or treatment planning, and monitoring; and advocates for clients and patients to resolve crises.

Other Information/Requirements: 

This position requires a associate’s degree (AA) or equivalent from an accredited Registered Nurse program and state licensure, and CPR certification. One year related experience, preferred in a community health care setting.
LFMC required training Including, but not limited to: basic Annual Safety, Bloodborne pathogen, Infection control, Hazardous Communication, HIPAA Training; Fluent in both English and Spanish, preferred.
