Main Areas of Responsibility:
Under general supervision, as part of a multi-disciplinary team, the Manager provides services relating to improving retention in care for specialized subsets of patients; ensures eligibility for a variety of medical and social services; and assists patients in completing appropriate paperwork.
Additional Areas of Responsibility:
Non-medical case managers may provide assistance in accessing social, community, legal, financial, and other needed services. This role may perform a range of administrative functions required to ensure efficient program operation.
This role requires a high school diploma or GED and 2 years of directly related experience. Verification of education and licensure will be required if selected for hire.
